fix(notifications): route Rocket.Chat links by monitor type

Build title_link from the monitor type (hardware -> infrastructure,
pagespeed -> pagespeed, uptime types -> uptime) and omit it when
clientHost is not configured, instead of always linking to the
infrastructure page.
